- Upon receipt of this booking enquiry form, I understand that On Stage Media will contact me to confirm details and availability.
- Once confirmation of attendance has been established, I understand that On Stage Media will send me a booking deposit which is non refundable. This, along with my signed booking form, will then secure the attendance of my photographer/s.
